Please include the following as subheadings in your paper and answer the components below each subheading in a paragraph or more:
Introduction:
A-In a paragraph using your own words introduce and describe in detail the scenario-based program you have selected to evaluate. Provide a rationale for why this program is important, what questions the evaluation will answer and how the evaluation will be used to make decisions.
b. Research the topic of your scenario using quality reliable health sources such as CDC.gov. In a paragraph or more define the health problem, the risk factors, prevalence, and any contributing behaviors.
c. Conduct a literature search (using google scholar, Pubmed or library databases) and find a journal article on a similar health promotion program. In a paragraph or more identify describe the similarity (How are they similar to your program?), the effectiveness (Were they successful?), and the limitations (What needs to be improved based off their results?) of both interventions. Does the article suggest areas for further research.
d. Create a logic model for your program scenario. Be sure to include all components including inputs, activities, outputs, impact and outcome. Describe / explain your logic model in a paragraph.
e. In a paragraph identify your key stakeholders and how they will be included in or effected by your project.
Methodology
a. In a paragraph identify and describe the evaluation design you will use to measure your objectives and evaluate your program. (Will it be qualitative, quantitative, or mixed method? If quantitative, will it be experimental, quasi-experimental, non-experimental, etc.? If qualitative, questions will be explored?). Include the design notation.
b. In a paragraph identify the specific program objectives that your evaluation will measure (process, impact and outcome), and describe what type of instrumentation will you use for data collection (will you use interviews, focus groups, observations, surveys, sign in sheets, etc.? Attach all instrument(s) you propose using for this evaluation in the Appendix. Organize this by objective. List the objective and identify what you will use to measure if it was achieved. You may list the same instrument / method for various objectives. Consider using a table to organize this section.
c. In a paragraph discuss any instruments that you propose using (i.e. survey). How was the instrument developed? What does it measure? Identify and describe how you will administer the instruments to reduce measurement error. Discuss the validity and reliability of your instruments. Was validity and reliability assessed? How can they be improved?
d. In a paragraph describe the population you would collect data from, the sample size you would use (how many participants will you collect data from?), and how the sample will be drawn from the population.
e. In a paragraph explain how you propose the collected data will be analyzed. Will comparisons be made? What will determine success? How will data will be organized for analysis and stored. What types of graphs or charts will you use to present results?
f. List any potential limitations of the proposed evaluation proposal (Are there threats to internal or external validity? Is the data generalizable? Can the design be made stronger?).

a. A minimum of 3 professional references should be cited in-text and in the bibliography.
b. All references, both in-‐text and bibliographic, should adhere to the American Psychological Association (APA) style.
Appendix
a. Instruments used for data collection should be placed here (focus group guidelines and questions, surveys, observational worksheets, spreadsheets, etc.)
