What are some challenges associated with translational research and how can you manage them?

Evidence-based practice (EBP) is a systematic research search reviewed, analyzed, and then translated into clinical practice. The end goal of EBP is quality patient outcomes. The use of EBP reduces healthcare costs and standardizes care for best practices. Translational research takes the rigorous research of EBP and translates it to practical application for real-world settings ​(Mazurek Melnyk & Fineout-Overholt, 2019)​. Translational research encourages a multidisciplinary approach and collaboration amongst laboratory and clinical professionals and community members. Identifies and supports best practice in medicine and health and focuses on the desires of the general public ​(University of Arkansas for Medical Sciences, (UAMS), n.d.)​, and this focuses on communities rather than the individual patients as in EBP.
Translational research has five levels of translational research, labeled T0 through T4. T0 is basic research in which definitions, mechanisms, targets, etc. are determined, and considered preclinical and animal studies. T1 is translation to humans, phase 1 clinical trials, in which new methods of diagnosis and treatment are used. T2 is translation to patients, or the phase 2 and 3 of clinical trials, where controlled studies lead to care. T3 is translation into practice, or the phase 4 of trials and clinical outcomes are reviewed. They will now become the recommended delivery of care for the patient population or community. T4 is translation to community. It is the last phase in which outcomes are now researched to see if it is a true benefit for society.
There are many ways to adapt translational research to society. Applications of translational research included targeted drugs for cancer treatment or more recently vaccine development for the coronavirus. This type of research can also include developing unique intervention strategies. For example, one of the local hospitals here in my area has started a community program to reduce violence in the community, especially among the younger generations. It is an at-risk program that links these people with community and hospital resources ​(UCHealth, n.d.)​.

Using the GCU Library: Nursing and Health Sciences Research Guide, I found a journal titled American Journal of Translational Research. The most recent issue contained an article titled, Effects of continuous nursing on rehabilitation compliance, living quality and daily living ability of patients with acute ischemic stroke, which caught my attention. In this article, they take the idea of continuous care and create a standardized nursing team to implement it on a small scale (n = 75) (Li et al., 2022). The use of translational research involves taking knowledge and applying it to practice. In the previous DQ, we discussed the steps of translational research, and from my understanding, this study falls into step 2 of translation research. Step 2 involves testing the efficacy of the new intervention. I also found a typing of translational research from T0 to T4. This classification system would place it at the T3, the translation of practice to compare effectiveness and outcomes (What Are the T0 to T4 Research Classifications?, n.d.). I’m not really sure what constitutes it being the best. I found both classification systems seemed like steps in a process to implement best practices, not separate pieces to pick from.
